Providing Access to Patient Records

This page gives information on how your practice can provide access to patient records if needed due to patient request or regulatory requirement.

Patient Portal

Patient Portal is a feature available to practices with a unique, active registration key. Through Patient Portal, patients can access the following information electronically:

Your practice can determine which features and information are accessible from the Patient Portal from Patient Portal Settings.

Access to the Patient Portal can be provided to a patient or their Authorized Representative. For more information, see Patient Portal Access.

Continuity of Care Document (CCD)

A Continuity of Care Document (CCD) is a health record document standard used to share patient health information. The CCD can be printed from Open Dental or shared electronically via the Patient Portal. See Continuity of Care Document for details on what information is included on the CCD.

Progress Notes

Progress Notes (e.g., procedure notes, etc.) are not included on a CCD and are not available in the Patient Portal by default.

Your practice can generate a copy of Progress Notes by doing the following:

  1. In the Chart Module, go to the Show tab.
  2. Choose a Chart View or set options to determine what information is displayed in the Progress Notes.
    • Only the information that is currently displayed in all pages of the Progress Notes area will be included in the copy.
      • To include all information saved to the Progress Notes, click the All button. To also inlcude deleted procedures, deleted sheets, deleted Chart letters, and procedure note history, check the Audit box.
  3. Click the Print button in the toolbar.
  4. Once the Print Preview generates (this may take a few moments), click Print.
  5. If prompted, choose the printer to use.
    • If you want to save a copy of the Progress Notes as a PDF, choose Microsoft Print to PDF, Adobe PDF, or a similar available option.
    • If you are not prompted to choose a printer, ensure the Default Printer in Printer Setup is set to Prompt.
