This page gives information on how your practice can provide access to patient records if needed due to patient request or regulatory requirement.
The following should satisfy most requirements of access to a complete patient record. Click a link to view more information:
Patient Portal

- Images and files in selected Image Categories

- Saved Treatment Plans
Your practice can determine which features and information are accessible from the Patient Portal from
Patient Portal Settings.
Access to the Patient Portal can be provided to a patient or their Authorized Representative. For more information, see Patient Portal Access.
Continuity of Care Document (CCD)
A Continuity of Care Document (CCD) is a health record document standard used to share patient health information. The CCD can be printed from Open Dental or shared electronically via the Patient Portal. See Continuity of Care Document for details on what information is included on the CCD.

- Once the Print Preview generates (this may take a few moments), click Print.
- If prompted, choose the printer to use.
- If you want to save a copy of the Progress Notes as a PDF, choose Microsoft Print to PDF, Adobe PDF, or a similar available option.
- If you are not prompted to choose a printer, ensure the Default Printer in Printer Setup is set to Prompt.
