eForms are an option for digital forms available for eClipboard users. Available for use in-office with eClipboard 25 and available online with eClipboard Web, eForms make data entry easier by prefilling fields for patients and automatically importing data for the practice.

Setting Up Forms

Open Dental comes preloaded with various forms useful for an office. These can be used right out-of-the-box, or you can customize them for your practice.

Branding
Rather than adding your logo to each individual eForm, instead, set up your eClipboard Branding Profile to add branding to eClipboard.

Automatic Import
Most eForm fields allow for automatic import. This allows data to be directly entered to the patient in Open Dental without the need for manual review. Automatic import is set on each desired eForm field by checking Auto Import. Information is imported to the patient immediately when the form is submitted.

Problems & Allergies
eForms allow patients to enter their problems and allergies using check boxes or radio buttons. These can be added directly to the Problems List and Allergies List in Open Dental upon form submission, or you can opt to review these before adding them to either list. Determine the default behavior for problems and allergies in eForm Setup.

Medications
eForms allow a patient to enter their current medications. To do this, ensure the Medication List field is added to a form.

Other Fields
Other fields on eForms include:

• Text boxes for additional fields, such as name and address.
• Labels to enter generic text, such as your HIPAA content or form explanations.
• A date field to pull the patient's birthday.
• Radio buttons for patient demographics, relationships, preferred contact methods, and more.
• Check boxes to allow for multiple item input.
• A signature box to obtain patient's signatures.
• Patient lists to easily add the primary or secondary insurance subscriber.
• The Insurance Pick list to select from existing insurance plans.

Conditional Logic
Conditional logic means a patient only has to fill the fields that pertain to them. For example, if a patient enters "No" to the question, "Do you have insurance", then all following insurance questions are hidden. Conditional logic is set by assigning a parent or child on each needed field.

Design
While most practices should be able to use the preloaded forms, you may need a specialized form for your practice. You can set these up yourself, or you can have Open Dental customize your forms for an additional fee.

Filling Forms

Patients can fill their forms in the office from an eClipboard 25 device. Alternatively, they can fill forms ahead of time from eClipboard Web.

Determining Forms to Fill
The forms and checklist items that automatically display for a patient use the Forms Behavior set in eClipboard Setup. Alternatively, specific forms can be added to a patient in Patient Forms by adding an eForm and marking the status in Fill eForm as Show in eClipboard.

Reviewing Data

To see what data was changed from an eForm, view the Audit Trail and set the permission to EFormImport. Each form logs the changes in the Log Text. View changes for all patients or select a specific patient.
