eForm Patient Lists allow for easy selection of other patients, such as insurance subscribers.

In eForm Def Edit, click Patient List.

Use the Patient List to easily select or enter the insurance subscriber when entering new insurance plans when filling eForms.

Label: Add a label to describe the Patient List box (e.g., Primary Insurance Subscriber).

Database Link: Where this information imports into the database. Select from ins1SubscriberPatNum or ins2SubscriberPatNum.

Required: Check to require this field be entered by the patient.

Show Border Box: Check to include a light gray border around the field.

Space Below: The amount of space below the field on a form.

Font Scale: Font is scaled to the device the eForm is viewed on.

Conditional Logic: This field will show...

• ... if the parent field: Click Pick to open the eForm Field Picker window.
• ... has the following value(s): Click Pick to select a value from the parent field (e.g., checked, unchecked).

Language Translation

When editing a translated eForm, the language translation fields display. For more information on editing translations, see eForm Language Translation.

What the Patient Sees

When in use on an eForm, the patient clicks Pick to select from other family members.

If the family member is not listed, the patient clicks Add to enter details.

Once entered, this patient is created in the database as a family member.

• This field does not consider existing patients in other families. It is possible to create duplicate patients in Open Dental when using this field. Duplicate patients can be combined using the Merge Patients tool.
