eForm Check Box Edit

eForm check boxes are used to connect problems and allergies to the database.

In eForm Def Edit, click Check Box.

Use Check Box settings to determine how a field behaves and interacts with other fields.

Label: Add a label to describe the Check Box (e.g., Aspirin, Pregnancy). Labels are bold when Labels Bold is checked in eForm Def Properties.

Database Link: Click the dropdown to determine if the data imports as a problem or allergy. Also determine if the address or phone information should be the same for the entire family.

Allergy

Only displays if allergy: is the selected Database Link.
Allergy: Click Change to open the Allergy Master List. Select the allergy that corresponds to the field.

To require patients select an allergy option, even if they have no allergies, add a field with a label of None and a Database Link of allergiesNone, then set the field as required. No information will be imported into the database. Optionally, create a eForm Text Box Edit with a database link of allergiesOther to allow a patient to type in an allergy not listed.

Problem

Only displays if problem: is the selected Database Link.
Problem: Click Change to open the Problem Master List. Select the problem that corresponds to the field.

Other Options

Layout: This section controls the appearance and behavior of a field. See eForm Field Layout for details.

Font Scale: Font is scaled to the device the eForm is viewed on.

Reportable Name: Give the field a unique name to be reported on in queries. Only used when there is no database link in use.

Locked: When checked, the patient cannot modify this field. Used when office is adding text that should not be modified, such as tooth number or treatment.

Auto Import: When checked, these fields are automatically imported to the patient upon submission.
